Working in the healthcare sector is under constant pressure. The nursing profession, already burdened by the complexity of care, comorbidities, and an aging population, has undergone numerous transformations in recent years. Recruitment challenges have added to the workload. There are more patients, shorter hospital stays, and consequently, more notes to write—notes that must be thoroughly documented and often written more quickly to keep up with demands. In addition to these new realities, there have been significant changes in the roles and scope of practice of healthcare professionals. Recent legislation on the administrative reorganization of care and the advent of new technologies have had a major impact on many tasks, including the writing of medical notes. Over time, through meetings, visits to healthcare settings in Canada, Switzerland, and Australia, and research, I quickly realized that the need for a documentation guide was shared by nurses everywhere. Mireille Guillemette is a registered nurse with over 30 years' experience and has been a member of the OIIQ (Québec Nurses Order) since 1993. She has worked in various healthcare settings, including CLSCs and several hospitals, such as Sacré-Coeur Hospital in Montreal, as well as several CHSLDs (long-term care facilities). She has also worked as a teacher, including a four-year period as an educational advisor. In 2013, she founded her own company, which specialises in accredited training. She travels throughout Quebec, o ering continuing education based on best practices and current knowledge. The training courses she o ers are accredited by several professional orders. She also works as a trainer for the Centre d'excellence du vieillissement de Québec (CEVQ), which is part of Laval University's integrated university health network. In 2019, she participated in an educational mission in Switzerland, training a team of professors and nurses to become mentors specialising in assessment and clinical intervention with seniors. Also in 2019, Mireille Guillemette designed a guide to writing nursing documentation for the OIIAQ (Québec Registrered Practical Nurses Order), and gave talks on the subject at regional conferences throughout Québec. This second edition has been enhanced with even more examples and digital content. Laws, standards, and regulations dictate the 'WHAT'. This guide will equip you with the 'HOW'.
